How to Refer a Patient
Submit a Referral
Submit a referral using our secure online referral form, or call our office directly with urgent cases or questions.
Send Supporting Information
Please include 1) the patient’s contact information, 2) relevant medical and dental history, and 3) any available radiographs or CBCT imaging.
We’ll Handle the Rest
Once we receive the referral, our front office team will contact the patient to schedule their consultation. We’ll coordinate closely with your office and keep you informed throughout the process.
